  • 2019 Form 940 and 1st Quarter 2020 Form 941 

  • 2019 Full Year Payroll Report

  • 2020 Payroll Report ⇒ 2/15/2020 (or nearest date)

  • Copy of SBA EIDL Loan Note (if applicable and only if you received funding from the SBA  after 1/31/2020)

If you do not have access to the above payroll verification documents, contact your payroll processor or accountant for assistance.
